To help carers during their visits, we have now introduced Incident Reports in the App with body maps.
How to create an incident report
1. Log in to the app and navigate to the client's profile (click on the name of the client)
2. Click on the "Incident Reports" window to create a new report. This new window will also show you if there are any open existing reports for your client.
3. To create a new incident report, click on the "New" button at the top right corner of the screen.
4. At the top of the screen, you will see the name of the client and the time of the visit in which you are recording the incident report, you can edit the date and time if needed. Make sure these details are correct before submitting the incident report.
You will then be prompted to select the type of incident, for example, Fall and include:
A title
Where did it happen
What happened before the incident
Details of actual incidents
You will need to type in or dictate these items to be able to submit your report

5. You can also include any additional events after the incident, add files, and use the body map if necessary. This is an optional step.ย
6. Submit the incident report once all the details are entered.
After submitting the incident report it cannot be edited or changed, so please make sure that all the information is correct before clicking ''Submit''
How to find all your existing incident reports
To easily access all the open existing reports, head over to your menu, click on Incident Reports and you will be able to see the full list for all of your clients (Alternatively you can open one of the visits of the client and navigate to the Client's profile to open the incident from there).
When an incident report is closed by an Admin or Manager, it will disappear from the carer's app. Only incident reports with status "Open" will be visible to carers.
How to add additional information to an existing incident report
To add more information to an existing report, click on the report and add an additional event or files.
Web portal view of incident reports submitted by carers
Web portal users (Admins, managers, etc) will receive a notification in the Notification Centre when a carer has submitted a incident report for a client. The incident report will be visible in the client's profile under the Incident Report tab.
Admins and Manager users are the ones that can close down an incident report when they consider necessary. They can also re open an incident report that was previously closed.
