The first step in creating your care plan is to build a template with the different categories and then in each client's profile you can then tailor this template to fit your service user's individual needs.
The template of the built-in care plan cannot be modified. If you have an specific template you can use the file attachment function to upload it to each client's profile and share this document with cares
Go to Admin menu > Settings > Clients, then select Care plan template. Only admin users can access these settings.
2. Use the Add new item button to add a new row to the care plan. You can think of each item as a type of category for care.
3. Give the item a descriptive title, e.g Personal Care, Medication, and so on. If there is standard information applicable to all your clients, you can pre-fill it into the template in these settings but remember that any information filled in here will be visible in every client's profile. Please don't add any client-specific info onto the template!
4. Then continue to add more categories your company will use in care plans. Later on, you can then remove the ones that do not apply for a particular client on their own profile page.
Important: If your categories follow an order use this numbering format before the names: 01, 02, 03 ...09, 10, 11 etc. This way the categories can be listed correctly. By default, the system will list the categories in alphabetical order.
5. Lastly, don't forget to click Save!
Transferring your care plan to a client's profile
1. After saving your template, go back to your client profiles. Select People > client name > Care plan:
2. You can now specify which categories are relevant for this client. If a section is not needed, you can delete the row or leave it blank.
3. Fill in any relevant sections to your client for each category. You can also add Tasks that are related to the specific care category.
4. Rate each category for priority as 'Non urgent', 'Needs attention' or 'Urgent'.
5. Once finished, save your care plan and lock it to input a review date. You will receive an alert in the Notification Centre 30 days before to the review date (you can decide how soon would you like to be notified in the Settings).
You can print or download saved care plans by selecting the print icon at the top right-hand corner of your finished care plan, select Print and save the PDF to share it or print it
Unlocking and reviewing care plans
On the review date, click 'Review' and add your notes before to select 'Review and unlock' and make the changes you need.
At this point, you can also add or remove information to the existing categories in your client's care plan, or you can remove entire categories that are not longer relevant. If needed, you can add new categories one-by-one from the Add item(s) menu:
Once you have made all the changes to the care plan, be sure to click Save and Lock in order to keep a record of the previous care plan and also to set a new review date.
At the bottom of the Care Plan tab, you will find the Care Plan History:
Task instructions
You can add client-specific instructions to the Tasks here in the Care Plan or in the Health tab in the client's profile.
To add instructions to a task:
1- Hover your mouse over the task name until you see the pencil icon:
2-Then click on it and add the client-specific instructions for that task and click Done:
Now the Task will have specific instructions for this client and the text will be visible in the Nursebuddy for Carers mobile app:
You can edit or remove these instructions at any time.
